Orthopaedic Surgical Associates provides patients multiple ways to request copies of their medical records. There is currently no charge to obtain your medical records through our patient portal, Athena Health. Please note, if you request a copy of your images, there is a fee and the CD must be picked up in the office.

You may also request a copy of your medical records through our vendor, HealthMark. There is a fee for this service.
